We’re here to make it easy to get the information you need to take charge of your health with free online access to most medical records. There are several ways you can request medical records:
MyChart
MyChart allows you to review your medical history (lab, test results, current immunizations, preventive care and medical/surgical history) securely. Once logged in, you can submit your request by clicking “Request my Records” in the MyChart menu.
Online
Hurley contracts with Datavant to fulfill requests for medical records. This secure portal allows patients and providers to request all or part of a medical record and it can be delivered electronically or through the US Postal Service.
