Hurley Heart Center combines compassionate care with the latest technological advances to address the community’s heart health. We say “community” because not only do we diagnose and treat heart problems, we enable healthy people to lower their chance of developing heart problems with workshops held out in the community to educate about risk factors, preventative measures and available treatments.
We also have locations in your community. A visit to our Heart Center gives you access to experts in every aspect of coronary heart disease. Whether you have a family history, need to keep tabs on a heart issue you’ve had since birth, or you develop symptoms out of the blue, our highly-trained, experienced cardiologists and staff can evaluate and treat your cardiac issues. That’s what makes us the preferred heart center for area providers.
Symptoms of heart problems can include:
- Chest pain, tightness, pressure
- Shortness of breath
- Pain, numbness, weakness or coldness in legs or arms
- Fluttering in your chest
- Pain in the neck, jaw, throat, upper abdomen or back.
If you have these symptoms, tell your Primary Care physician right away. You’ll be referred to one of our convenient clinic locations to receive on-site EKGs, cardiac monitors, echocardiogram services–any test necessary to diagnose your heart problem.

What Happens At Your First Heart Clinic Appointment?
We’ll start out by discussing your family health history and addressing any questions you might have about your symptoms. We’ll perform a thorough physical exam to check your blood pressure, weight and current heart function, and may conduct other simple tests on systems that could affect your heart health, such as a blood screening to check cholesterol levels.
We may order an echocardiogram, which uses sound waves to create detailed pictures of your heart, allowing us to get a good look at your heart’s chambers, valves, walls and blood vessels. This helps us check for coronary artery disease (atherosclerosis), aneurysms or blockages in the arteries.

Hurley’s Congestive Heart Failure Nurse Navigator
A congestive heart failure patient’s outlook for the future depends on factors such as how well they respond to and follow their treatment plan. With this in mind, Hurley offers a congestive heart failure nurse navigator program to help patients and their caregivers navigate the healthcare system. The nurse navigator’s clinical know-how and support allows patients to focus on feeling better and managing their condition successfully.
What is the Role of Hurley’s Nurse Navigator?
The congestive heart failure (CHF) nurse navigator assists patients at the bedside when they are hospitalized and with consistent follow-up beginning 1-2 days after discharge. Nurse navigators make sure patients understand discharge instructions, which helps decrease the worry many feel upon returning home. Providing personalized care that continues for about a month, the CHF nurse navigator helps coordinate follow-up appointments and maintains contact with primary care physicians and other members of the care team, keeping everyone in the loop. The navigator checks in with patients every 7-10 days to ask how they are feeling and give them emotional support, even providing a number for them to call with questions or concerns.
With the CHF nurse navigator to empower patients and families, connecting them with expertise in every stage of their journey to recovery, it’s no surprise that CHF readmission rates are improving.
