New Patients

If you’re a new patient at Hurley Mental Health Associates:

• Please print out the forms listed below, fill them out and bring them to your first appointment.
• You should be at the clinic 40 minutes before your appointment to complete paperwork.
• Patients who complete these forms ahead of time may arrive 15 minutes early.

• Patients 18 and older
• Patients 17 and younger

Forms for Release of Information

• Authorization for Release of Information
• Authorization for Communication with Primary Care Provider
