Non-Discrimination Policy
In accordance with Hurley Medical Center’s policies, ANPPS is willing to assess and treat all patients as long as the referral purpose is an appropriate match for our service providers and practices. We do not discriminate with regard to race, color, ethnicity, sex, pregnancy, marital/parental status, age, religion, disability, sexual orientation, or gender identity/expression.

Referrals & Intake Process
There are two paths of referral to ANPPS for new patients.

  1. You may have been seen at Hurley Medical Center during an inpatient admission, and the Physician e recommended you follow up with the clinic after discharge.
  2. You, your family or your physician may feel you would benefit from ANPPS services. For any testing, the clinic requires a referral from your physician, often PCP or other specialty medical provider be faxed to the office at 810-239-1281. The referral should specify the services requested and the primary purpose. Once the information has been received, the providers will review it and determine if the referral is appropriate for this specialty clinic. If the referral is appropriate, the office will contact you by telephone to obtain additional basic information such as contact information and insurance. The initial appointment would be scheduled and an intake packet mailed to you or you would be instructed as to how to download the forms from the website so they may be completed prior to the intake appointment. Medical records would also be requested so if you have any, please bring them to the first appointment.

Fees

ANPPS is an approved provider for many local and national health insurance plans.

Covered Insurance Plans
Due to the various services provided, ANPPS staff will work with you to verify that services will be covered by your health insurance plan on an individual basis. For some insurance providers, we can complete an initial appointment (intake), but authorization must be requested and granted for additional services.

Doctor’s Referral Required
In order to utilize HMO/insurance billing for testing, a physician referral is required and can be sent to the office fax number at 810-239-1281.

Verify Your Insurance
Even though ANPPS has been approved as an in-network provider with a variety of insurance carriers, you may still be responsible for out-of-pocket payments, depending on your individual plan. Please contact your insurance carrier for an estimate of benefits because ANPPS does not verify individual insurance benefits. It is the responsibility of each patient/guarantor to ensure services are covered PRIOR to initiation of treatment / assessment. If you don’t have insurance, please contact the clinic for an estimate of out of pocket fees.

Forms

A number of forms must be completed prior to the first appointment. Click on the button associated with the patient’s age to print then complete the forms. Releases of information should be returned to the office prior to the appointment if possible, so we may begin to request needed information.

FAQ’s

How long will the assessment take?
The initial appointment will be between 1-2 hours long. The testing may take 2-6 hours. The final feedback appointment is typically an hour but could take longer.

How much will it cost?
Cost varies depending on the amount of testing done, and the amount that your insurance carrier reimburses and the deductibles and copays that are required of your carrier.

Who gets the results?
The referring physician and the patient or in the case of a minor, parent will receive a copy of the results.
